Are these white or gray?

They are white with gray grips to hold the clothes on the hanger

Does this hanger prevent shoulder bumps? I bought some of the velvet kid’s sized hangers, but they are awful for uniform polo short shoulders.

They are good and don’t cause irregular bumps. I hang my kids polo uniform shirts on them.

Has anyone had issues with the hook coming out under normal wear conditions?

I hang my adult (small) clothes on these hangers and sometimes the closet is very tight. I have never had any of these have the hooks come off. It happens often with other brands but never has with these.

If stepped on, will they break?

I doubt it. I haven't tried, but these are great quality. Very sturdy. Great size too.
